School Forms

The following grades have required screenings. Students will need to have the following forms returned to the school office before the first day of school. If you wish to use the Religious Exemption Form, Bethany, School Secretary is a Notary to stamp and sign your form.

TK & Kindergarten

Kindergarten Physical (Include Lead Testing at physical) *Religious Exemption Form may be used for Lead Testing

Immunization Record and/or Religious Exemption Form

NOTE: If you turned in these forms for TK and continue on at SCS for Kindergarten, you DO NOT need to repeat or turn in again.
