The following grades have required screenings. Students will need to have the following forms returned to the school office before the first day of school.

Kindergarten

Kindergarten Physical (Include Lead Testing at physical) *Religious Exemption Form may be used for Lead Testing

Dental Screening

Immunization Record /Religious Exemption Form.

New Student Any Grade

Health Physical Form

Immunization Record and/or religious exemption form.

3rd Grade

7th Grade
